The answer depends entirely on how many of the three sites you want and whether you are going under the arena. This page is the real timings — not the listing durations, but how long each version of the visit actually occupies of your day once you add the arrival, the screening and the walk between sites.

The standard guided tour of the Colosseum, Roman Forum and Palatine Hill takes about two and a half hours — call it three and a half of your day with arrival and screening. A tour of the amphitheatre alone runs one to two hours. Underground tours run from seventy-five minutes to three hours. And a combined day with the Vatican takes seven to ten. Add half an hour before. Add as much as you like after — the ticket is still valid.
Every duration on every listing measures the tour, from the moment the guide starts to the moment they stop. Around that sit three pieces of time that are just as real.
Before: thirty minutes. You are asked to arrive twenty to thirty minutes before the slot so the group can assemble, collect headsets and clear security together. That is not padding; a group that arrives at the printed time does not get in at the printed time.
Getting there: fifteen to thirty minutes. The Colosseo metro station is directly outside, which is unusually convenient, but Rome traffic is not, and being late for a timed slot is a real problem rather than an inconvenience.
After: as long as you want. The most useful thing on this page. Your admission does not expire when the guide stops talking. Most tours finish on the Palatine Hill, inside the site, and you can stay. Between the guide leaving and the park closing there is often another hour or two of the best light and the thinnest crowd of the day.
So a two-and-a-half-hour tour is a half-day commitment, and it can easily be a very good five hours if you let it.
